TERMINATION PROCEDURES
The franchiser, [Name of the franchiser], and the franchisee, [Name of the franchisee], as set forth in their franchise agreement on [Date of the agreement] and as witnessed by [Name of the witness], have agreed to the following additional terms and conditions: [Description of the terms and conditions agreed upon]
Upon termination of contractual agreement the franchisee will perform the following:
A. Stop conducting business immediately at [Address of franchise]
B. Stop the use of the following trademarks or logos:
1. [First Trademark or logo of the franchise]
2. [Second Trademark or logo of the franchise]
3. [Third Trademark or logo of the franchise]
4. [Fourth Trademark or logo of the franchise]
C. Telephone numbers for the franchisee must be transferred to the franchiser or to [Party the phone should be transferred to] or canceled immediately.
D. All the following materials must be returned to franchiser immediately: [Materials to be returned to the franchisor]
E. Any and all contracts relative to franchise must be provided to the franchiser immediately.
If the franchisee does not comply with terms and conditions of termination immediately [he/she] will allow the power of attorney to execute all necessary acts to complete termination process.
